Bus Form 2026-2027
Below is the updated bus form for the 2026-2027 school year. You are required to complete and turn in this form if your child currently rides or is planning to use the public bus services for the next school year.
Please follow the instructions below carefully to ensure your students transportation is secured:
- Download and save the 2026-2027 Bus Form PDF under your student's first and last name
- Fill out the document completely with your current address (First and last names only, middle names should not be included)
- Send the completed PDF to our schools facilities coordinator, Mr. Lara at julara@charterk12.com
THIS FORM MUST BE FILLED OUT AND SENT DIGITALLY IN PDF FORMAT- PRINTED/SCANNED FORMS WILL NOT BE ACCEPTED.
